An exciting opportunity has arisen in the Community Diabetes Specialist Team, for an enthusiastic, innovative Nurse. We are a friendly, forward thinking team looking for the right person to complete our team. To act as a resource and role model in expert practice facilitating and supporting the delivery of direct specialist diabetes patient care for patients and all members of the Primary health care team, nursing and residential homes.

All new staff will be subject to a probationary period covering first 6 months in post. Travel between hospital sites may be required.
Be the responsible lead in diabetes service development within the locality. To evaluate service delivery, identify areas for improvement and plan future development.
To provide Specialist Diabetes Triage/ Assessment clinics within the community setting.
