We are looking for an experienced Practice Nurse to work with a friendly team based in Conwy.
Your role will involve providing excellent care for our patients and be responsible for the delivery of services, working as part of our multi-disciplinary team.
Main duties of the job
Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
Provide wound care (ulcer /Doppler etc.) to patients
Request pathology services as necessary
Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
Maintain accurate clinical records
Ensure read codes are used effectively
Implement vaccination programmes for adults and children
Support the clinical team with all safeguarding matters, in accordance with local and national policies
Deliver opportunistic health promotion where appropriate
Provide baby immunisation
Provide cytology
Manage emergency drug kit

Meddygfa Gyffin and Llys Meddyg Surgeries, based in Conwy, are part of the Llanfairfechan Group Practices.
We use EMIS Web and AccuRx providing services to 6,500 patients.
Our highly skilled clinical team comprises of 4 GPs, Advance Nurse Practitioner, Community Clinical Practitioner, Practice Nurse, Physiotherapist, HealthCare Assistant, Phlebotomist and a Pharmacy Technician. All are fully supported by our experienced Care Navigation and Admin teams who work across both sites in Conwy.

Job responsibilities
Provide nursing treatment under the GP direction.
This can include and is not limited to: management of chronic diseases (following recall procedure), cytology screening, childhood/travel immunisations, complex wound dressings i.e. ulcers/compression, dealing with minor injuries/wounds, some phlebotomy, ECG etc.
Provide health screening with agreed protocols with referral to GP as necessary
Advise patients on general health care and minor aliments with referral to GP as necessary
Act as chaperone to GPs as necessary
Undertake the collection of pathological specimens including intravenous blood samples, swabs, smears, on the spot urinalysis(dip) testing, and perform any investigatory procedures requested by the GP.
Ensure that all monitoring processes for patients with chronic diseases are undertaken in accordance with practice policy and in a timely manner.
Ensure accurate notes of all consultations and treatments are recorded in the patients notes/computer records Assist in the formulation of practice philosophy, strategy and policy.
Liaise with the reception staff to ensure forms and office procedures are maintained
Ensure the adequate maintenance of equipment and stock levels in the treatment rooms, liaising with the Practice Manager
Person Specification
Qualifications
* NMC Registered Nurse
* Diploma in at least 1 or 2 CDM areas e.g. Diabetes, CHD Relevant post-graduate study.
Experience
* Clinical observations
* Sample collections
* Injections and administration of medications
* Childhood Immunisations
* Experience in at least 1 or 2 CDM areas e.g. Diabetes, CHD, COPD, Asthma Cervical Cytology Communication skills verbal and written.
* Well-motivated.
* Positive attitude to work and willingness to learn.
* Able to work under pressure
* Ability to manage own workload and work independently.
* Ability to provide quality care excellent interpersonal and communications skills
* Ability to communicate in welsh
* Experience of community working Audit process
* Experience using EMIS clinical systems
