Reference: 05ffb6db PA for elderly lady with Mixed Alzheimer and Vascular Dementia Support tasks needed: Fortnightly to put black bin out on a Thursday, this person will not put the recycling out until Friday morning due to being concerned that the contents will blow around/get wet, also keeping up to date on the rubbish bags being taken from the kitchen and the back porch out to the black bin. Weekly look at Fridge to monitor the contents for out of date products Help to clear out clutter and dispose of expired food Weekly hygiene clean around kitchen surfaces etc Take this person to town for a cup of tea and social engagement. This person feels down and feels lonely sitting in her home most of the time. PA availability schedule: Monday - 9.00 - 10.30 - Breakfast, washing and dressing. Tuesday - 10.00 - 14.00 - work in the kitchen - Fridge take out the food which is out of date, clean the fridge, look at the freezer - defrost and take the items which are out of date. Check the food in the cupboards, try to unclutter kitchen. Help iron the clothes. Help to have a bath. Monitor and write down fluid intake while at the property. Wednesday - 16.00 - 18.00 - Support with preparing the supper, monitor food and fluid intake, please record it for the social worker to see it. Help with the dishes. Change the bedding, put the clothes into washing machine. Thursday - 10-12 Grocery shopping, support with grocery shopping, community engagement. Help to put the bins out, try to unclutter second room. Friday - 16.00-18.00 Support with preparing supper, monitor food and fluid intake, please record it. Help with the dishes, clean the kitchen work tops. Saturday - 10.00-13.30 - Support with breakfast, shower and dressing. Clean the kitchen, bathroom (top services and the floor). Sunday 11-13.00- Help to go to the church. This lady is socially isolated. She will require a PA to build a trusting relationship with, so that she will eventually agree to more care and support. Having some support and interaction is vital to this ladies wellbeing as well as important to maintain her physical and mental health.
