If so, this is an exciting opportunity to join our Older People / Physical Disabilities (OP/PD) Adult Social Care team in Norwich.
We are seeking enthusiastic Assistant Practitioners to join our highly motivated team of Social Workers, Occupational Therapists and Assistant Practitioners. You will be part of a service that is embracing new ways of working, including our Community Social Work model, Human Learning Systems and Community Led Support (CLS). These approaches focus on early intervention, creativity and making the most of community assets to prevent, reduce and delay the need for formal care.
About The Team
You will be joining a passionate, supportive and high‑performing OP/PD team with extensive experience working with complex cases.

We are proud of our multidisciplinary environment and our positive working relationships with Health colleagues. We work closely with GP surgeries, Integrated Care Coordinators (ICC) and the Living Well teams to ensure people receive the right support at the right time.

To build meaningful relationships with the people we support, this role requires regular in‑person work in both the office and community.
What You Will Be Doing
* Undertake Care Act Assessments, Support Planning and Reviews
* Contribute to our duty system, responding to new enquiries and supporting people at the earliest opportunity
* Work collaboratively with partners, including health colleagues, community organisations and voluntary sector services
* Support people to access universal and preventative services
* Promote independence, resilience and wellbeing in line with our Living Well agenda
* Engage with people in community spaces as part of our Community Led Support model
* Build strong relationships with individuals and families to achieve positive outcomes
You will also have opportunities to work on smaller project areas, including:
* Supporting our holding list, ensuring preventative measures are in place while people wait.
* Working alongside GP practices, ICC teams and Living Well services to strengthen community connections.
* Helping shape and expand our community‑led support offer across Norwich.
