Job Summary
Shropshire Community Health NHS Trust are excited to recruit Assistant Practitioners to work as part of the Care Transfer Hub (CTH) Team based across the Shrewsbury and Telford Hospitals to support complex discharge.
The hub brings together staff from the NHS, Shropshire Council, Telford & Wrekin Council, and the voluntary and community sector. Together, these multi‑organisational professionals facilitate safe and timely discharges, ensuring appropriate support is in place for each patient.
Since its launch in October 2024, the hub has helped to significantly reduce the number of patients waiting in hospital beds for discharge into home or community care settings. It has shortened the average wait times for discharge and advocates a "home first" philosophy, supporting patients returning home from hospital.
By enabling multidisciplinary teams to coordinate discharge planning effectively, the hub identifies the best and safest options for patients who are medically fit to leave hospital. This collaborative effort involves social care, housing and voluntary organisations working in partnership to ensure patients receive the timely support they need.
Main duties
* To take responsibility for a defined group of patients following delegation by the Registered Health Care Professional.
* Work under direct and indirect supervision of a Registered Health Professional to provide health care support to an identified caseload—this may be in an inpatient or community setting including the patient's own place of residence.
* To work within agreed protocols within the designated work area.
* Maintain agreed, acceptable standards of care and only undertake duties within own personal competence following training/education and assessment of competency.
* Undertake triage process and prioritising patients.

Qualifications
* Essential: Foundation Degree in Science (FdSC) in Integrated Care OR Assistant Practitioner Level 5 course modules and clinical competencies OR Assistant Practitioner Health Higher Apprenticeship OR Equivalent qualifications.
Experience
* Essential: Demonstrable experience of working within a clinical setting.
* Experience delivering personal and clinical care.
Knowledge and Skills
* Essential: Good verbal and written communication skills.
* Good IT skills and computer literate.
* Ability to develop knowledge and skills in providing healthcare support, evaluation of treatment plans and reassessment of care within agreed protocols.
Other
* Essential: Ability to meet travel needs of the post.
