Come and join our small friendly team delivering excellent patient centred holistic care to our Parkinson's disease patients across the Fylde coast. The successful candidate will have a keen interest in Parkinson's disease and will provide expert clinical care, education and support to our patients and families living with Parkinson's disease. The post holder will work autonomously and as part of a supportive multi disciplinary team to deliver patient centre care that promotes independence, symptom management and enhances quality of life for our patients and their families.
Deliver comprehensive assessment, diagnosis support and ongoing management of individuals living with Parkinson's disease.
* Develop, implement and review plans of care in collaboration with patients, families and other healthcare professionals.
* Monitor motor and non-motor symptoms, medication effectiveness and potential side effects.
* Conduct home visits and community clinics.
* Act as a point of contact for patients, carers, GP's and other healthcare professionals supported by the Parkinson's disease team.
* Provide psychological support to patients and carers adjusting to diagnosis and disease progression.
* Advocate for patients to ensure access to appropriate services, benefits and support networks.
* Contribute to the development of Parkinson's disease services across the Trust.
* Participate in audit, research and evidence based practice initiatives.
* Maintain accurate documentation and comply with professional and regulatory standards.
As a member of the Parkinson's disease team the successful candidate will be supported by our Lead Consultant in Parkinson's disease and our Nurse consultant. You will also have the support of our wider Care of the Older Person department.
