We are looking for an experienced and qualified practice nurse to join our team, based in Ilkley with potential cross-site work at our smaller practice in Grassington.

Main duties of the job

To support the Infection Prevention Control (IPC) lead by providing audits, training, support, and evidence of compliance to the practice manager.

To develop, implement, and embed an effective care management program for frail and housebound patients registered at the practice.

To assess patient needs ensuring healthcare provision is appropriate, incorporating evidence-based practice.

To develop, implement, and embed health promotion and wellbeing programs.

To implement and evaluate individual treatment plans for patients with chronic diseases that promote health and wellbeing.

To identify, manage, and support patients at risk of developing long-term conditions, preventing adverse health effects.

To provide routine nursing care to patients in accordance with clinical evidence, NICE, and NSF guidelines.

To provide wound care (ulcers, Doppler, etc.) to patients.

To oversee chronic disease clinics, delivering patient care and referring to secondary or specialist care as needed.

To maintain accurate clinical records in accordance with legislation.

To ensure effective use of SNOMED CT codes by the nursing team.

To assist GPs with minor surgery procedures when required.

To support the team in managing clinical emergencies.

To implement vaccination programs for adults and children.

To deliver opportunistic health promotion where appropriate.

IG Medical is located in West Yorkshire, with two sites: Ilkley Moor Medical Practice in Ilkley and Grassington Medical Centre in the Yorkshire Dales. We are a training practice with a forward-thinking, caring, and committed team dedicated to improving community health through high-quality patient care.

Our clinical team includes GP Partners, salaried GPs, Advanced Clinical Practitioners, Practice Nurses, and Healthcare Assistants, supported by management, reception, and administrative staff.

Job responsibilities

To manage patients with long-term conditions using evidence-based practices, including care for elderly and housebound patients.

As a senior member of the multidisciplinary team, ensure effective delivery of nursing services to the patient population.

The Practice Nurse will oversee areas such as infection prevention and control, health promotion, chronic disease management, and clinics for well women and men, as well as support clinical policy review and implementation.

Ensure access to clinical supervision and support from a designated experienced individual within the organization.


Person Specification


Qualifications

* Registered Nurse (Nursing and Midwifery Council)
* Wound care/removal of sutures and staples
* ECGs
* Venepuncture
* Immunisations (routine, childhood, travel)
* Chronic disease management in respiratory conditions
* L2 Diabetes qualification or working towards it
* Mentor or teaching qualification
* Postgraduate diploma or degree in Chronic Disease Management
* Nurse prescriber
* Family planning qualification


Experience

* Experience working in a team of nurses and HCAs
* Experience in a GP practice setting
* Excellent communication skills (written and oral)
* Effective time management and organizational skills
* Ability to work autonomously and as part of a team
* Good interpersonal skills
* Problem-solving and analytical skills
* Ability to follow clinical policies and procedures
* Polite and clear telephone manner


Disclosure and Barring Service Check

This position is subject to DBS checks in accordance with the Rehabilitation of Offenders Act 1975.

Salary: £19 to £23.32 per hour plus additional benefits.
