Overview
Clinical Triage Sister, Integrated Single Point of Access (ISPA) - 18.5 hours per week. We are seeking experienced clinicians with a nursing qualification and an interest and/or experience in triage to join our Integrated Single Point of Access Team. The role is being established as part of our transformation programme and will involve a multi-disciplinary workforce alongside our administration team. The service operates across various clinical bases 7 days a week with a 24-hour shift pattern; roles are office based with some clinical time, and flexible working is available to support work life balance.
Responsibilities
* Triage and prioritisation of all new referrals.
* Prioritisation of urgent community response and hospital at home referrals.
* Apply expert clinical knowledge and judgement to facilitate decision making.
* Prioritise referrals through the clinical triage service as necessary and provide senior clinical input to ensure patients are signposted to appropriate services.
* Support the provision of the Spa MDT and ensure effective outcomes for patient contacts with the clinical triage services.
* Adapt to the changing needs of the Trust and community models of care as they arise.
Qualifications and Experience
* Nursing qualification.
* Keen interest and/or experience in triage and clinical decision making.
* Sound clinical knowledge and risk assessment skills.
Working Arrangements
The post holder will be office based with some clinical time included. The service operates across multiple clinical bases with a 7 days per week, 24-hour shift pattern. The role offers flexible working arrangements to support work life balance.
Development and Training
