About the role
The role of a Shared Lives Coordinator is to manage a caseload of carers who support people with learning disabilities in their own home. You will offer ongoing support to the carers through coaching, mentoring and advocating on their behalf. You will lead on several initiatives including training packages, support groups and new assessments. Working as part of a small team, you will understand what it takes to deliver an outstanding service to the people we support, and their carers based on person centred thinking and approaches.
Duties will include:
- Core Accountabilities to the People You Support and their carers.
- Making sure their support is delivered in a person-centred way, balancing up what is important to and important for that individual.
- Making sure all records relating to the individuals on your caseloads are up to date and accurate.
- Building a strong working alliance with the carers within the service, keeping in regular contact with them, responding to their needs, regularly giving them feedback, and auditing the quality of their work.
- Using the shared lives matching tools to ensure the people we support are matched with the most suitable people to meet their needs.
About you
To be a Shared Lives Coordinator at Certitude, the following are essential:
- Have a track record of enabling choice and control to be a reality for people with learning disabilities.
- Knowledge of KLOEs in the context of CQC quality standards.
- Ability to assess, support, coach and motivate people and carers.
- Experience of supporting people to positively engage as citizens within their community, cultivate relationships and be an active member of the local community.
- Experience of producing written reports to a high standard.
