Mental Health & Wellbeing Practitioner Based in Bolton’s Mental Health Support Team (MHST)

MHST Clinical Lead and BLGC Service Manager
Contract:
Fixed Term until March 2026. Hours: 37 hours/week.
About the role:
An opportunity to work as a practitioner in the national Mental Health Support Team (MHST) programme across Bolton schools and colleges. Your role involves supporting children and young people with mild to moderate mental health issues, using evidence-based interventions, and helping parents understand mental health better. You will promote a Whole School Approach to mental health and wellbeing, working closely with school staff to identify needs and foster a positive mental health ethos. The role aligns with the iThrive model, providing timely, young person-centred support.
Main Responsibilities:
* Deliver outcome-focused, evidence-based interventions for children and young people with mild to moderate mental health problems in educational settings.
* Collaborate with schools to embed a whole school approach to mental health and wellbeing, providing feedback and suggestions for creating mentally healthy schools.
* Record, develop, and deliver intervention plans with young people.
* Assess and manage risk collaboratively, developing safety plans and liaising with relevant services.
* Coordinate with health and social care staff from various agencies to support young people, ensuring integrated care plans, and participate in multi-agency meetings.
* Evaluate intervention success regularly using Routine Outcome Measures (ROMs).
* Work with young people and families to access appropriate support across health, social care, and VCSE sectors.
* Ensure services are accessible and responsive to BAME, LGBTQ+, and other minoritized young people.
* Maintain standards of safe practice.
* Guide, advise, and signpost to relevant services, recording all work undertaken.
* Document assessments, plans, interventions, and outcomes, maintaining case notes on communication, meetings, and activities.
* Ensure the safety of children and young people in line with BLGC policies and safeguarding arrangements.
* Stay updated on legislation related to children and young people.
* Participate in reflective supervision both individually and as part of the team.
