Overview
Location: Whiston Hospital, Nightingale House. An opportunity has arisen for a driven and innovative Assistant Team Manager to join Integrated Health and Social Care in the Front Door Service within Contact Cares. This large and busy multi-disciplinary team is the single point of access for health and social care referrals. The team includes qualified and unqualified Social Care, Nursing and Therapy staff. We offer planned and urgent assessments and provide a 2-hour response under Urgent Community Response Criteria.
The Contact Cares team provide initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. The role involves working with a focus on prevention, early intervention, and reducing hospital admissions and readmissions. The team operate from 8.00 am until 10.00 pm, 7 days per week including Bank Holidays. This post will be shift working to a rota over 7 days.
Responsibilities
* Support delivery of the Front Door Service within Contact Cares, acting as a key point of contact for health and social care referrals.
* Supervise and develop the multidisciplinary team, including social care, nursing and therapy staff.
* Coordinate initial contact, care planning, onward referrals, and signposting for adult clients.
* Ensure adherence to Urgent Community Response Criteria and maintain 2-hour response times where applicable.
* Promote prevention and early intervention approaches to reduce hospital admissions and readmissions.
* Contribute to roster planning and ensure effective service delivery across shifts.
Qualifications and Requirements
* Must be able to drive, have access to a car and hold a valid licence.
* This post is subject to Enhanced DBS Disclosure.
* Experience in health and social care settings, with supervisory or team management experience preferred.
