About the Role As a My Way Facilitator (Transitions Co-ordinator), you will walk alongside young people and their families as they move into, through, and out of our No Limits provision our bespoke further education programme. You will: Be the first point of contact for families at the pre-placement stage, creating warm, trusting relationships from the outset. Work closely with families and professionals to support person-centred transitions that reflect each young person s unique hopes and needs. Attend and contribute to EHCP reviews, ensuring that every young person leaves us with meaningful progress and a clear, ambitious next step. Collaborate with our team to help new learners settle and thrive in their provision. Own your schedule, mainly working weekday hours with occasional flexibility to meet family needs in evenings or weekends. This is a community-based role, based primarily around Abingdon and the Oxfordshire area, with regular travel to surrounding counties. Why Join MacIntyre? IND1 About you We're seeking someone who brings: Experience in education, health, or social care, particularly supporting children and young people with learning disabilities and/or autism. A deep understanding of the challenges families face during times of transition particularly the cliff edge that often comes post-16 and a solution-focused, compassionate approach to supporting them. Knowledge of EHCPs and how to implement them in real, empowering ways. Excellent project planning and IT skills, and the ability to coordinate effectively with a wide range of professionals. A person-centred mindset, aligned with MacIntyre s DNA : warm, respectful, creative, and always led by what matters most to the individual.
