Overview
The Central and East Locality in Exeter City is a community nursing hub covering 11 GP Practices in Exeter City and surrounding areas. Based at Whipton Hospital, Exeter.
Role purpose
You will be responsible for delivering care to patients in their own homes and in clinic environments, providing holistic nursing assessment, nursing care, and information to patients and carers within home or other appropriate settings. You will work as part of a wider team to prevent hospital admissions where possible and to facilitate discharge so patients are cared for in their own home where appropriate, supporting independence and quality of life.
Responsibilities
* Provide clinical care and holistic nursing assessment for patients in home and clinic settings.
* Deliver nursing care, advice and information to patients and carers; support discharge planning where appropriate.
* Work with the wider team to prevent hospital admission and optimise patients’ independence and quality of life.
* Collaborate with other health and social care professionals as part of integrated patient care.
Qualifications and experience
* Experience in community nursing is desirable but not essential as full training will be provided.
* Full driving licence and access to a car with business insurance is required to undertake the role.
* Lone working is expected after induction with ongoing support from coordinators and triage facilitator.
Working pattern
08:30 to 16:30. 7 days per week; part-time role available.
