An exciting opportunity has arisen in to support the medicinesoptimisation in Great Eccleston Health Centre, Lancashire. We are recruiting an experienced Clinical Pharmacist,part time (Between 22.5 - 30 Hours per week negotiable).

Requirements of the role include:

At least 3 years post graduate experience

Excellent Clinical knowledge

Self-motivated and able to work autonomously,whilst also integrating into a well-established team.

Excellent organisational, interpersonal andcommunication skills together with a good working knowledge of primary care andthe challenges facing medicines management in the new era.

The ability to adapt to be able to conveyinformation to patients and fellow healthcare professionals.


Main duties of the job

The Clinical Pharmacist will play a pivotal role in improving patientcare, increasing practice efficiency, and supporting the safe and effective useof medicines. The post holder will be responsible for prescribing leadership,improving clinical safety, reducing GP workload, and contributing to thedelivery of QOF and CQC standards. The role includes both patient-facing andstrategic responsibilities within a multidisciplinary team.


About us

Great Eccleston Health Centre is located on the Fylde Coast, Lancashire. We are a friendly 3 partner practice accompanied by an on-site pharmacy with a patient population of 7800.

The Practice is part of Lancashire and South Cumbria ICB and works alongside one other practice as a rural neighbourhood with a combined population of approximately 20K.

Our clinical team consists of three GP Partners, two salaried GPs, two NPs, a team of practice nurses and HCAs, a Pharmacy technician and an onsite community team. The successful candidate will be supported by an innovative practice who fully recognises the potential of the Clinical Pharmacist role to meet the changing demands of general practice.


Job responsibilities

Key Responsibilities

1. Clinical and Patient Care

* Undertake structured medication reviews, especially for patients with polypharmacy, frailty, or multiple comorbidities.
* Conduct face-to-face and telephone clinics for long-term conditions (e.g. Diabetes, Asthma, COPD, Hypertension, Heart Failure, Depression, RA).
* See patients with minor ailments and manage same-day appointments where appropriate.
* Review and action discharge and clinic letters, ensuring medicines reconciliation and safety.
* Lead on prescribing: audits, reauthorisation, medication enquiries, patient concordance, repeat prescribing systems.
* Support chronic disease management in joint clinics with clinicians.
* Provide vaccinations where within scope.

2. Prescribing and Medicines Optimisation

* Write, implement, and monitor protocols and policies on prescribing, storage, administration, and disposal of medicines.
* Lead on ensuring medicines optimisation targets of contracts are met.
* Oversee the practice formulary and ensure alignment with LMMG and NICE guidance.
* Monitor shared care and high-risk medications (e.g. DMARDs, lithium).
* Lead initiatives to reduce waste, improve outcomes, and enhance medication safety.
* Respond to drug safety alerts and ensure compliance with MHRA and national guidance.
* Liaise with hospital and community pharmacists for continuity of care and shared care arrangements.
* Repeat prescribing and monitoring.

3. Quality, Safety, and QOF

* Lead CQC-related workstreams and audits relating to medication safety and governance.
* Maximise QOF performance through proactive audits and patient reviews.
* Lead on Near Patient Testing returns and coordinate submissions for LES activity.
* Review unplanned admissions and implement strategies to reduce medicine-related harm.
* Support antimicrobial stewardship and lead on relevant audits.
* Maintain up-to-date EMIS templates and reporting tools to optimise clinical workflow.

4. Education and Leadership

* Provide expert medicines advice to GPs, ANPs, nurses, HCAs, and non-clinical staff.
* Educate patients about medication changes, safety, and adherence.
* Support the induction, training, and ongoing development of clinical and dispensary staff.
* Develop and maintain prescribing policies and contribute to practice strategy and service development.
* Lead and contribute to research, audit, and clinical trials where appropriate.
