Job Summary
The Care Coordinator plays a pivotal role in ensuring the effective and seamless delivery of high-quality care to clients in their homes. This individual acts as the central point of contact for clients, their families, and caregivers, and is responsible for organizing, scheduling, and monitoring care services to meet individual needs and maintain compliance with all relevant regulations. The Care Coordinator is a highly organized, compassionate, and detail-oriented professional who contributes to the overall well-being of clients and the success of the home care service.
Key Responsibilities
Care Planning and Coordination:
* Work closely with clients, their families, and other healthcare professionals to develop, implement, and review personalized care plans.
* Assess client needs and ensure that all support services are delivered in line with the care plan.
* Proactively identify and address a person's changing needs, making necessary adjustments to their care plan.
* Act as a communication bridge between clients, families, and caregivers, ensuring everyone is informed and aligned on care goals.
* Scheduling and Staff Management:
* Manage and create effective and efficient care rotas and schedules for a team of caregivers.
* Match new clients with suitable caregivers based on skills, personality, and location.
* Handle scheduling conflicts, last-minute changes, and on-call duties to ensure continuity of care.
* Support and supervise caregivers, addressing any concerns and providing guidance and feedback.
* Participate in the recruitment, training, and supervision of care staff.
* Communication and Administration:
* Maintain accurate and up-to-date client and caregiver files, including care plans, progress notes, and communication logs.
* Handle incoming inquiries and correspondence from clients, families, and professionals.
* Ensure all administrative tasks are completed in a timely and accurate manner.
* Report any issues, concerns, or violations to management immediately.
* Compliance and Quality Assurance:
* Ensure all care services are delivered in compliance with company policies and relevant health regulations, such as those set by the Care Quality Commission (CQC).
* Conduct regular oversight visits and checks to assess the quality of care being provided.
* Collect feedback from clients and families to ensure satisfaction and identify areas for improvement.
* Contribute to risk and impact assessments and participate in quality improvement initiatives.
Required Skills and Qualifications
* Experience:
* Proven experience in a care coordination, scheduling, or similar role within the health or social care sector, preferably in a domiciliary or home care setting.
* Experience in managing and leading a team of care workers.
* Skills:
* Exceptional organizational, time management, and problem-solving skills.
* Strong verbal and written communication skills with the ability to communicate empathetically and effectively with a diverse range of people.
* High level of IT proficiency, including experience with scheduling software and record-keeping systems.
* A compassionate, patient, and proactive approach to care.
* Ability to work well under pressure and handle difficult situations calmly.
* Demonstrated ability to maintain confidentiality and professional conduct.
* Qualifications (Desired):
* A relevant qualification in Health and Social Care (e.g., NVQ/RQF Level 3 or higher).
* Full UK driving license with access to a vehicle.
