The Carers Intensive Support Team is looking for a hard‑working, dedicated and compassionate individual to join us in supporting the health and wellbeing of those caring for a person with suspected or confirmed dementia, frailty, or an older person with mental health concerns.
In this role you will support family carers to look after their physical, mental, emotional and social health, enabling them to maintain their caring role and build resilience for the future.
You will be required to work across Southend, Castle Point & Rochford (South East Essex) to attend Carers Groups and Events and to cover annual leave and sickness absence.
A valid full UK driving licence and access to a car are essential, as the role requires travel across South East Essex.

Main duties of the job
Based in the Older Adults Mental Health & Dementia Service and working with Frail+ partners to identify rising carer burden and take action before carer breakdown occurs for those in the most deprived areas across South East Essex.
Identify and support older carers who are living with long‑term conditions and who neglect their own health due to their caring role, including those whose physical and mental health is deteriorating because of deconditioning, strain of caring, poverty or digital/literacy deprivation.
Complete Health and Wellbeing Checks for carers and use FrEDA on SystmOne to support early identification of respiratory disease, hypertension, deteriorating mental health and other long‑term conditions.

Job responsibilities
Work with EPUT Community Teams – including Older Adults Mental Health, Dementia Localities, DIST, Primary Care Networks (PCNs), Local Authorities (LAs) and Voluntary / Third sector services – across Southend, Castle Point & Rochford to support vulnerable and ageing carers (over 65).
Identify and support opportunities for Carers Health and Wellbeing Checks to:
* Ensure early identification of respiratory concerns.
* Ensure carers uptake of flu, pneumonia and other vaccinations to prevent hospital admissions and maintain their caring role.
* Discuss lifestyle choices such as smoking, diet, inactivity that may contribute to increased risk of cancer.
* Undertake or refer for BP, pulse and other physiological monitoring.
* Identify rising carer burden in deprived areas and take action before carer breakdown.
* Support disadvantaged carers by addressing literacy or digital exclusion.
* Assist carers facing fuel and/or income poverty, digital and literacy exclusion and deprivation.
* Use Carer Health checks to identify lifestyle choices that increase cancer risk.
* Identify those at risk of isolation and loneliness; facilitate support to minimise this.
* Encourage older adults with mental illness or concerns to seek help from OPCMHT and access NHS Talking Therapies for common mental health issues such as anxiety & depression.
* Educate carers on available services and opportunities to access social and therapeutic activities to reduce isolation and loneliness.
* Support carers with understanding the importance of contingency planning.

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving the population's health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ITS initiative between EPUT and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care Assistant (HCA) workforce.
To support this workforce model the successful candidate for this post will be required to attend a 5‑day (Monday to Friday) face‑to‑face Induction Programme at our MSE‑ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of the two HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process.
Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and to provide opportunities to enhance skills, knowledge and experience.
Further information will be provided post‑interview.
This will be followed by an additional 5‑day (Monday to Friday) EPUT corporate / mandatory training welcome. EPUT Welcome will be varied between face‑to‑face and virtual delivery.
Person Specification
Education and Qualifications
* Student Associate Practitioner (band 3), prepared to undertake study or have relevant experience.
* Full UK driving licence & access to own car.
* Relevant qualification or experience in Community Development / Education, Health or Social Care.
Experience and Skills
* A good understanding of the needs of people with dementia, who are frail or in the last 12 months of life and their carers.
* Treating patients & their carers / family with respect and dignity.
* Methodical, organised approach to work with good attention to detail.
* A good understanding of older adult mental health issues and their treatment/management.
* Knowledge of Trust IT systems and software packages, including Mobius and SystmOne.
