Service Team Leader – Clinical Coordination Pathways

About the Role We are seeking an experienced and proactive Service Team Leader to oversee the delivery of a specialist clinical assessment and treatment pathway within a growing healthcare service. This role is ideal for someone who excels in operational coordination, patient communication, and service governance, and who is confident leading a pathway from initial referral through to ongoing medication management.
You will play a key role in ensuring patients receive a safe, efficient and well-structured experience, working closely with clinicians, prescribers, pharmacy partners and internal teams. As the service expands, this position will evolve into a leadership role managing a team of Patient Coordinators.
Key Responsibilities :
Patient Pathway Coordination Manage the full patient journey from initial triage to assessment, treatment initiation and ongoing review.
Conduct structured triage calls, gathering relevant history, risk indicators and suitability for the service.
Allocate cases to clinicians based on complexity, capacity and governance requirements.
Oversee booking of assessments, follow-up reviews, medication appointments and ongoing check-ins.
Provide clear, supportive communication to patients, families and referrers.
Medication & Prescription Coordination Coordinate initiation, titration and stabilisation processes in line with clinical pathways.
Liaise with prescribers to ensure timely and accurate issuing of prescriptions and supporting documentation.
Manage prescription requests, repeat cycles and pharmacy-related queries.
Maintain detailed logs of medication changes, safety updates and compliance matters.
Quality, Governance & Patient Safety Ensure full adherence to clinical pathways, safeguarding frameworks and incident-reporting procedures.
Monitor quality indicators across triage, treatment and review processes.
Promote a positive patient experience and advocate for safe, accessible care.
About You
The successful candidate will have:
Experience in healthcare operations, clinical coordination or a similar service-delivery environment.
Strong organisational and communication skills, with the ability to manage complex caseloads.
Confidence working with multi-disciplinary teams, including clinicians and pharmacy partners.
A commitment to patient safety, service quality and continuous improvement.
Leadership capability with the potential to supervise and develop a growing team.
