As an In-Reach Recovery Coordinator, you will develop strong partnerships and joint working within the hospital, community health services, and beyond to actively improve services, seek to reduce hospital admissions and improve health and well-being outcomes for people. This role is to engage with complex service users who are engaged in a cycle of hospital admission due to several unmet needs. You will look to assess immediate housing needs, work with partners within health services and the wider community. You will work in collaboration with service users and relevant partners to promote and empower social integration. This is an 11 month Fixed Term Contract. Where: Hastings Hours: 37.5 Salary : £ 27,449.53 - £ 31,529.41 About the role: Supporting service users from point of entry into the service and through their treatment/recovery journey Providing screening, assessment, and recovery planning and onward referral Reducing drug and alcohol related harm to service users and the wider community Promoting carer, service user and community involvement Providing advocacy for access to partnership services Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination About you: Have a strong understanding of substance misuse issues and experience of working within a similar field Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
