Role overview

Oldham Council has an exciting opportunity for a motivated and enthusiastic person to take up the role of Social worker grade 6 within the Learning Disability and Autism Team. The successful candidate will work as part of a supportive team to provide assessments of need, care and support planning, and review, that takes a strength based approach, with individuals who may have a range of social care needs, their families, and informal carers


Key responsibilities

* To manage a caseload of service users. You will need to be able to plan, assess and review an individual’s needs in line with the Care Act.


* To identify and take account of community networks and assets, which may assist in meeting the needs of individuals and their carers who are referred for a Care Act Assessment or who are in receipt of services,


* To work with individuals and families with health and social care needs that require innovative approaches and models of support to promote independence and improve social inclusion alongside your social work colleagues and other agencies.


* To organise and/or assist to obtain the provision of appropriate services to meet identified needs, minimise risk, offer protection and maximise independence appropriate to the Care Act eligibility criteria risk levels and to keep providers advised and informed of any changes to care arrangements. To ensure all relevant systems are updated in a timely manner and all case notes are contemporaneous and accurately recorded.


* To consult with service providers in monitoring the effectiveness of care plans as required. To undertake reviews/re-assessment visits, when necessary, in line with the Directorate’s policies and procedures and to re-evaluate the needs of individuals and their carers to ensure that services are adequate, effective, appropriate and of a high quality standard.


Key Requirements

* Experience of working with individuals with Learning Disability and Autism.


* Excellent communication and engagement skills are required to effectively advise and support service users and their Carers.


* It is essential you are able to understand the principles of safeguarding or be prepared to undertake the appropriate training.
