Advanced Clinical Practitioner or Advanced Nurse Practitioner

PELC is seeking an enthusiastic Advanced Clinical Practitioner or Advanced Nurse Practitioner with independent prescribing to work with us across our sites. You will deliver face‑to‑face services, enabling patients to obtain health information from the appropriate health care professional. You will be providing safe and effective health assessments and treatment of patients within the establishment.

You will be accountable for your practice including the assessment, diagnosis, treatment, and discharge or referral of patients, independent of medical intervention to provide high‑quality support for professional practice within the organisation.


Main duties of the job

The post holder will be an experienced caring nurse/practitioner who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries. You will work within a clinical team consisting of GPs, UCPs, Nurse Streamers and HCAs to deliver excellent care services within our UTC. You will communicate at all levels across boundaries and work closely with wider clinical team members. The post holder will also work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures.

You will be an integral member of the clinical team, completing streaming, independently assessing patients and prescribing clinical intervention. You will also be completing administrative tasks (prescriptions, referrals, pathology results).

As an ANP/ACP you will be supported by, and work with, a diverse multi‑skilled professional team of clinical and non‑clinical friendly staff, providing high quality, person‑centred, holistic clinical care to patients.


Clinical practice

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition (as appropriate).

Diagnose and manage both acute and chronic conditions, integrating both drug‑ and non‑drug‑based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence‑based practice and national and practice protocols, and within scope of practice.

Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over‑the‑counter medication on medication regimens, side‑effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self‑care.

Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.

Promote and deliver evidence‑based care for patients presenting for opportunistic wound care.


Communication

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Anticipate barriers to communication and take action to improve communication.

Maintain effective communication within the practice environment and with external stakeholders.

Act as an advocate for patients and colleagues.


Delivering a quality service

Recognise and work within own competence and appropriate professional code of conduct as regulated such as NMC and HCPC.

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, code of conduct, policies and procedures.

Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

Deliver care according to NSF, NICE guidelines and best evidence‑based care.

Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.

Evaluate patient's response to health care provision and the effectiveness of care.

Support and participate in shared learning across the practice and wider organisation.

Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patient health procedures and local guidance.


Professional Practice

Demonstrate and provide supportive professional practice for clinical development of student as required.

Promote, support and embed the vision, values, and goals of PELC throughout all professional practice.

Demonstrate fairness and respect as central to excellent professional behaviours, and lead and influence others to do the same.

Support service improvement plans to achieve the best outcomes for patients.

Support patient safety and governance processes to ensure improvement in service delivery.


Autonomy/Scope within Role

Assumes clinical accountability for patients referred into non‑medically led services.

Practices competently and autonomously without direct supervision in accordance with professional code of practice and local policies and procedures.

Acknowledges limitations in own professional competence and only undertakes practice and accepts responsibilities for those activities for which competence can be assured.

Exercises professional judgement and expertise to create precedents and deal with ambiguous or novel problems.

Applies best practice policy guidelines where appropriate and in the absence of specific guidance ensures that actions are initiated in line with evidence‑based practice and rigorous analytical judgement.

Collaborates with other relevant professionals to develop clinical protocols and integrated care pathways within area of expertise or speciality to benefit the delivery of clinical services.

Influences the culture of the environment to ensure both the atmosphere and surroundings meet the needs of individuals and humanises patient care.

Monitors and leads improvements to standards of clinical care through supervision of practice, clinical audit, evidence‑based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.


Person Specification


Qualifications

* *Registered with NMC or HCPC with advanced clinical practice/Advanced Nurse Practitioner qualification
* *Nurse‑led/HCPC management of minor illness, or minor ailments and injuries
* *Evidence of post‑registration study at undergraduate level (Level 6 or 7 in minor illness)
* Experience of clinical and educational programmes
* Relevant Education Qualification (PG Cert Clinical Education or equivalent)
* Master degree in advance clinical practice


Skills

* Knowledge of clinical governance framework
* Ability to write and present information effectively and concisely
* Broad knowledge of professional, educational and other health care policy
* Knowledge / experience of development advanced clinical skills / roles
* Ability to deliver training to groups and individuals
* Ability to complete audits and root cause investigation
* Ability to lead on improvement projects
* Proven experience in leading, managing and working in a multi‑professional team
* Demonstrates an understanding of the principles of research


Experience

* Knowledge of current clinical issues and the changing landscape of the NHS
* Experience of, understanding of managing professional development
* Good understanding of regulatory requirements from the Care Quality Commission, and other regulatory bodies
* Experience working in a fast paced urgent care treatment centre or emergency department
* Experience in clinical incident investigations
* Experience in completing clinical audit and other quality improvement methodologies
* Highly developed persuasive, motivational and negotiating skills.
* Clinical experience working with older people
* Experienced mentor, appraiser or clinical supervisor
* Contributes to quality improvement
* Experience in working in Emergency/Urgent or Primary Care
* Understanding of the principles and practice of knowledge of clinical audit and evaluation of services.
* Demonstrable record of partnership working and stakeholder engagement
* Evidence of participation in clinical supervision and Professional Nurse Advocate programme
* Excellent understanding human resources policy and practice
* Experience of programme design and project management in the implementation of workforce development strategies
