Health & Wellbeing Practitioner - SuttonPermanent£25,714.00Join The Forward Trust and make a real difference. We deliver integrated clinical and psychosocial substance misuse services across Surrey Prisons, working in close partnership with NHS healthcare providers. Our trauma-informed, gender-responsive approach ensures every service user gets the support they need on their recovery journey.About the Role:As a Health & Wellbeing Practitioner, you’ll be part of a multidisciplinary team delivering recovery-orientated services to women aged 18+. Working on-site at HMP Downview, Sutton, your role will involve: Managing a caseload with a person-centred approach Conducting assessments and co-producing care plans Delivering structured 1:1 sessions and facilitating group work Providing harm reduction advice and support Performing second signatory duties Supporting release planning and community reintegration Accurately recording data and outcomes to measure successRequirements: Experience or interest in working with individuals affected by substance misuse or low-level mental health needs Strong commitment to trauma-informed and gender-responsive practices Ability to work collaboratively across prison, healthcare, and mental health teams Willingness to work weekends on a rota basis Commitment to completing the Drug and Alcohol Workforce Development Programme (DAWFDP) within 12 monthsBe part of a service that’s changing lives and supporting positive futures.Please note SurreyHealth & Wellbeing Practitioners are now working 37.5 hours as a 12-month‘New Era’ pilot scheme.
