About the Role
Care Workers provide safe and personalized support to citizens based on their assessed needs. This includes writing and maintaining individual person-centered care plans, conducting assessments, and regular reviews. They also plan, organize, and facilitate individual or group activities/interventions to support citizens in maximizing independence and confidence.
Care Workers collaborate with other professionals, citizens, and their meaningful others to ensure a shared approach to care and support, ensuring continuity of care and treatment.
Care Workers at Level 3 should maintain up-to-date knowledge of relevant policies, procedures, and legislative requirements related to the role, client group, and Adult Social Care. They are also expected to support Care Workers at Levels 1 and 2 in providing safe, personalized care, and to handle safeguarding incidents if needed.
About You
The ideal candidate will have:
* Ability to assess social care and health needs, develop, implement, and update Person-Centered Care Plans, and utilize various manual and IT systems effectively.
* Excellent oral and written communication skills, including facilitating reviews, engagement, report writing, maintaining good records, and handling difficult conversations. Awareness of Health & Safety requirements and risk assessments.
* Knowledge and understanding of the rights, needs, and feelings of vulnerable adults from diverse social and cultural backgrounds.
* Good numeracy and literacy skills equivalent to Level 1 literacy Skills (Qualifications and Credit Framework) or an equivalent qualification.
* NVQ/QCF Level 2/3 or equivalent in Health and Social Care, or willingness to work towards this qualification.
Note: A DBS enhanced check is required for this position. This role does not meet the salary threshold for Skilled Visa sponsorship; applicants requiring sponsorship cannot apply.
