Job summary: A holistic approach WorkWell Workshop to support early intervention, in partnership with wider stakeholders such as VCSE, local authorities, community services, mental health and social sectors.
A core MDT panel will facilitate the running of the workshops consisting of:
* Physiotherapist led exercise advice
* Pain cafe - low level pain management strategies delivered by pharmacist
* Social prescribing for community support (e.g. isolation, activity, financial advice, housing)
* Mental health and wellbeing check in
* Impartial HR/employment advice for managing sickness and return to employment
* Health coach offering health check lite focussing on physical lifestyle interventions (blood pressure check, weight management, smoking cessation, exercise referrals)
Main duties of the job: Low level interventions within community workshops for patients who are currently employed but on sick leave having 2 or more fit notes issued in the last 6 months

Qualifications Essential:
* Experienced Physiotherapist available to work up to 1.5 days per month
