Job summary
To support the Primary Care Network to ensure the delivery ofsafe, effective and efficient systems for repeat prescribing, medicinesoptimisation, reducing medicines waste and maximising patient outcomes.
The role will also include training and development ofadministrators to support improved co-ordination and effective pharmacyadministration within general practice, especially with relation to repeatprescribing systems and processes.
Main duties of the job
To undertake medicinesreconciliation in accordance with practice protocols and to update patients’medical records / prescribing accordingly. To support the achievement ofthe practice’s prescribing Quality and Outcomes Framework targets.Working with the Practices tocontribute to the review and development of repeat prescribing protocols /Standard Operating Procedures to improve the efficiency and effectiveness ofrepeat prescribing systems. To provide advice to GPs, staffand patients in changes to prescribing to support the improvement ofprescribing safety, quality and cost participate in medication assist in the appropriatemonitoring and management of prescribing budgets and to liaise with themedicines management team at the prepare evidence basedresources and information to support the implementation of rationalcost-effective prescribing. To support the Practices toprovide medication reviews, patient information leaflets, medicine awareness;and to undertake community visits as take a proactive approach tothe safe handling of prescriptions, including assessment of risk and makingrecommendations for respond to medication queriesfrom patients and staff in a professional manner referring to the appropriateGP in accordance with practice protocols. To advise on the sourcing andsafe management of medicines as appropriate.
