Mindworks Hub Screening and Triaging Clinician

Main duties of the job
Provide timely screening and triage of children and young people where Mindworks has received a request for support for their emotional wellbeing/mental health needs, including offering discovery conversations where ensuring the safety of the child/young person may be a priority.
Work individually and jointly within the Mindworks Surrey alliance to support children and young people to access the most appropriate care for their needs.
Confidently signpost using the iThrive framework, considering where needs may not be based in mental health and where support can be sought from a range of Surrey systems partners.
Job responsibilities
The post holder will be integral to the functioning of the Mindworks Surrey Hubs, responding to requests for support for emotional wellbeing/mental health needs.
Work as an autonomous practitioner, undertaking clinical screening and where appropriate assessment, of children and young people who may have a range of needs, including more complex comorbid mental health needs or may require urgent support.
Co‑coordinate and liaise with the Mindworks Hub team to ensure flow of new requests are appropriately discussed and considered.
Work within a highly experienced multi‑disciplinary team involved in the assessment and treatment of children and young people with a range of mental health difficulties.
Manage a caseload as part of the Mindworks Hub team, from receipt of request to transfer to other services or closure, ensuring there is appropriate information to enable and inform decision making.
Ensure that outcome measures are considered, embedded and audited; contribute to relevant research and ensure that interventions on the pathway are evidence‑based; raise concerns and gaps in pathways to the Service Manager to enable planning and continuous improvement of flow and accessibility.
Accurately record all activity as per professional and directorate standards, including risk summaries, risk assessment and care planning. Formulate a robust My Safety Plan with young people and their families where required.
Contribute to the development and implementation of clinical guidelines, protocols relevant to sphere of responsibility as well as continuous improvement initiatives.
When demand and capacity allow, support team members to meet the demands of the service.
Person Specification
* Two years post‑qualification experience of working in the community with children and young people with emotional wellbeing and/or mental health difficulties and their families
* Experience and confidence in undertaking mental health assessments, risk assessment, implementing risk management plans and embedding outcome measures
* Experience in working in a multidisciplinary team
* Awareness of the iThrive framework
Qualifications
* Degree level qualification in Nursing, Social Work or Occupational Therapy, with associated professional UK registration (NMC, SWE or HCPC)
