End of Life & Bereavement Specialist Nurse
The role of the End of Life & Bereavement Specialist Nurse is to take a dedicated role in the implementation of the SWAN model of care. The SWAN end of life and bereavement model is patient and family focused, centering on meeting the unique needs of each individual and their loved ones at the end of life and after death. It aims to provide excellent, individualized care to every patient and their families, including the provision of immediate and ongoing emotional support to the recently bereaved throughout the Trust and Merseyside area.
The responsibilities include advising, educating, and implementing the model throughout the Trust to ensure compliance with statutory duties for SWAN, End of Life, and Bereavement care, as stipulated in primary legislation, government strategy, and relevant guidance.
This role also involves staff supervision, practice development, and case work management across the Trust, ensuring clinical staff are competent in this specific area of practice.
Key responsibilities include:
1. Meeting the unique needs of each individual and their loved ones at the end of life and after death.
2. Working autonomously, making critical decisions and judgments related to the management of End of Life, Bereavement Care, and the SWAN model, including report writing to support these decisions.
3. Communicating sensitive and complex information in health and multi-agency forums.
4. Promoting a proactive approach to SWAN, End of Life, and Bereavement Care across all services.
5. Working in partnership with all involved agencies and organizations by attending relevant meetings and subgroups to ensure strategic planning and service development.
6. Developing, implementing, and evaluating policies, standards, and procedures related to Bereavement and SWAN care.
7. Contributing to annual reports for the SWAN, End of Life, and Bereavement models to the Quality Committee and Trust Board.
8. Providing training around SWAN, End of Life, and Bereavement Care in line with national and Trust guidance.
9. Participating in clinical audits and research related to end of life and bereavement care.
The role is within Liverpool University Hospitals NHS Foundation Trust, comprising Aintree University Hospital, Broadgreen Hospital, and Royal Liverpool University Hospital, part of the NHS University Hospitals of Liverpool Group. The group aims to improve patient care, serving over 16,800 colleagues and approximately 630,000 people across Merseyside.
The Trust's hospitals offer a range of services including trauma care, elective surgical and diagnostic services, specialized regional services, and maternity services, including the UK's largest single-site maternity hospital.
Additional responsibilities include:
* Supporting the development, implementation, and evaluation of policies, standards, and procedures to meet national, regional, and local legislation and guidance.
* Ensuring a robust, compliant Bereavement and SWAN referral process.
* Utilizing tools like the Liverpool Quality Assessment (LQA) to monitor and improve SWAN, End of Life, and Bereavement Care services.
* Supporting quality and performance standards compliance, including CQC requirements.
