Crown House Surgery is seeking two experienced ARRS-funded Advanced Paramedics. Both positions are full-time, 37.5 hours per week, working over five days with coverage until 6:30 pm.
The successful candidates will be enthusiastic and motivated, working autonomously within their scope of practice to manage minor illnesses, long-term conditions, and exacerbations of chronic illnesses such as COPD and heart failure. They will play a crucial role in the daily operations of the surgery, including managing acute and semi-acute illnesses and conducting up to two home visits per day.
Applicants must hold relevant qualifications with their professional bodies.
The practice boasts a wide multidisciplinary team, including nurses with expertise in chronic disease management, treatment room skills, pharmacists, pharmacy technicians, and a social prescriber.

Main duties of the job
* Registered with HCPC with advanced clinical practice.
* Previous experience in general practice or primary care is essential.
* Undertake comprehensive clinical assessments and prescribe autonomously.
* Manage acute and chronic conditions, conduct assessments, and make appropriate referrals.
* Manage undifferentiated and undiagnosed conditions, identify red flags, and take necessary actions.
* Make critical clinical judgments, including first contact management of acutely presenting patients and managing care with or without protocols.
* Make direct referrals to hospital specialists when appropriate.
* Use complex decision-making to inform diagnosis, investigations, and management within a broad scope of practice.
* Adopt a personalized and population-centered care approach to facilitate shared decision-making.

The practice is part of Retford & Villages PCN, with roles employed via Fidelium Health (FHL). The positions are fully embedded within the practice, with HR management handled locally. Employment contracts will adhere to the practice's policies and procedures.

Person Specification
Qualifications
* Qualification in Advanced Physical Assessment & Consultation Skills (APACS) & minor illness modules or equivalent.
* Registered with HCPC as a paramedic.
* Proficient in clinical IT systems.
* Masters degree or equivalent in Advanced Clinical Practice with digital badge.
* Knowledge of various IT systems, including word processing, email, and internet-based applications.
Experience
* Experience working in a primary care setting or Primary Care Network.
* Ability to listen empathetically and provide person-centered support non-judgmentally.
* Courteous, respectful, and helpful demeanor.
