Band 3 Operational Discharge and Flow Support HCSW – a role within the Hampshire Hospitals NHS Foundation Trust focused on improving patient flow and supporting early discharges from the wards and emergency departments.
Responsibilities
* Coordinate discharges with the Integrated Discharge Team, Patient Flow Facilitators, ward teams and the Operations Centre.
* Maintain accurate data on electronic whiteboards (eWhiteboards) and use Power BI dashboards to gather discharge intelligence.
* Manage transport arrangements, ensuring patients have safe and timely transfer to the discharge lounge and subsequently to their destination.
* Engage with patients and their families, communicating clear expectations and supporting them through the discharge process.
* Support earlier movement from the emergency department and assessment areas, relieving blockages and improving bed availability.
* Escalate barriers promptly to Band 7, Band 5 or the Duty Manager and work collaboratively with stakeholders to resolve them.
* Redeploy rapidly to respond to operational pressure and anticipate evolving site requirements.
Person Specification
* IT literate.
* Secondary Education qualification, or equivalent knowledge and skills; NVQ level 2 or above in Health or Health and Social Care, or equivalent experience.
* GCSE Standard Pass Grades – grades 4 and above or A‑C (Department for Education: June 2017).
* Previous NHS experience preferred.
* Previous experience in a Health/Health and Social Care role and awareness of discharge processes across the acute hospital setting.
* Ability to undertake the physical requirements of the role.
Additional
* Ability to redeploy rapidly to areas of greatest operational pressure.
* Physically capable of the demands of the positioned role.
