An exciting opportunity awaits a dynamic and motivated individual to join our Medical Director Services Team. We are seeking someone with exceptional interpersonal and communication skills to act as the vital link between GP practices and SaTH medical staff, fostering strong partnerships and ensuring seamless patient pathways while strengthening operational links between the Trust and primary care.

The postholder will be the point of contact for GP Practices and SaTH medical staff seeking to resolve queries, concerns and complaints related to patient pathways between primary and secondary care. The postholder will be responsible for maintaining the contact management database, administering and coordinating queries, ensuring effective responses to feedback are focused, addressing issues raised and recording full details on the contact management database.


Responsibilities

* Responsible for maintaining the contact management database, administering and coordinating queries, ensuring effective responses to feedback are focused, addressing issues raised and recording full details on the database.
* Be the primary point of contact between the Trust and Primary Care with regard to medical colleagues, to establish operational links between both.
* Receive and resolve queries, concerns and complaints related to patient pathways between primary and secondary care, including but not limited to fit notes, onward referrals, clinic letters, discharge summaries and prescription/investigation requests.
* Receive and make telephone calls to/from GPs, Consultants, wards and other NHS professionals, following through all enquiries in a friendly and professional manner.
* Effectively liaise with GP practices and departments within the Trust, developing efficient working relationships.
* Problem‑solve on complex, contentious, sensitive and emotive areas, using tact, diplomacy and empathy in conveying information that might be contested or where there are barriers to understanding and acceptance.
* Undertake regular discharge summary audits in conjunction with GP colleagues in primary care.
* Communicate complex and sometimes contentious pathway information to a wide range of stakeholders, and provide support to both clinical and administrative staff to resolve and correct patient pathway issues.
