We are looking for three Lead GPs to cover the three Integrated Neighbourhood Teams in the areas of Surrey where CSH provides community services. Each role will be for two sessions a week and will be mainly based in the locality that the GP is leading.

You will be responsible for working with the current neighbourhood teams to provide excellent care and support for the local community. You will have a key role in developing the local teams and services to match the needs and priorities of the communities. The role requires GPs with a passion to improve place-based care and for building the relationships that this requires across many organisations. You will be a problem solver and enjoy the variety of challenges that this role will bring.


Main duties of the job

The main duties will include the delivery and management of the locality INT MDTs as the current focus for developing neighbourhood services. The post holder will work closely with the Operational Leads to develop place-based services that are identified as the priorities for the community including co-development with stakeholders. A key part of this role will be developing strong relationships across many organisations that make up the Integrated Neighbourhood Teams (INTs).

Key duties will also include responsibility for service mobilisation and agreeing and reporting the services key performance indicators (KPIs). Some budget management will also be required. Hold responsibility for the clinical quality, safety and governance of contracted services.

An overall duty will be the responsibility for the clinical quality, safety and governance of the neighborhood services that the postholder is directly involved with.

Job responsibilities

* Post Title: Lead GP for Integrated Neighbourhood Teams - CSH Community Services
* Salary: Depending on experience
* Business Unit: Neighbourhoods
* Responsible to: Medical Director for CSH
* Accountable to: Chief Operating Officer

Purpose

Pivotal to our success will be the Lead GP for Borough, Integrated Neighbourhood Teams (INTs). Reporting to the Medical Director/COO, the GP Lead will be responsible for the overall clinical and service delivery of community services within an identified Borough set of neighbourhood teams. Alongside the, this includes line management of appropriate neighbourhood clinical staff as well as joint responsibility for all clinical and non-clinical neighbourhood staff, responsibility for the management of resources and financial management and achievement of KPIs and outcomes. Lead GPs will take a lead role in supporting the development of the wider INT ways of working, service transformation and place-based care.

The role contains two key components, operational delivery plus the development and transformation of new services as they are integrated into the Neighbourhoods of NW Surrey


Person Specification


Qualifications

* MBBS or equivalent undergraduate degree
* MRCGP or equivalent
* Full GMC registration and Licence to Practice
* CCT in General Practice or equivalent
* Hold a clinical post for a practice in the relevant geographical area
* Registered on the Medical Performers List or appropriate professional equivalent
* Up-to-date training on the following: Adult Safeguarding, Basic Life Support, and Equality and Diversity Training


Skills

* Ability to listen to the needs of others/active listening
* Collaborative and enabling leadership style
* Excellent oral and written communication skills Presentation skills

Experience

* Experience of having worked as a clinician in a primary care environment
* To be able to consult independently, assess and manage patients at first presentation
* Experience in working in Frailty, Care of the Elderly, caring for people with dementia
* Evidence of up-to-date knowledge of: Management of Long Term Conditions, Mental Health including Dementia, Health Promotion, and End of Life Care
* Understanding of current issues and challenges within health care delivery in CSH and the wider NHS
* Understanding of quality and safety improvement, and clinical governance principles applied within clinical settings
* Awareness of own scope and limits of practice
