As a Community Support Worker you will act as a key worker for customers, assisting with the provision of positive enabling intervention and support for adults with mental health problems, physical and/or learning disabilities.
What will you be doing?
* Provide practical and emotional support to help individuals maintain independence.
* Assist with daily living activities, including personal care and medication (as per care plans).
* Support customers in accessing community resources, services, and benefits.
* Work as part of a team managing a small caseload of individuals.
* Contribute to care planning, reviews, and accurate record keeping.
* Promote wellbeing, safety, and independence at all times.
18.5 hours per week.
Shift schedule:
* 7am - 3pm with alternate weekends of 3pm - 10pm and 8am - 3pm.
* 8am - 3pm with alternate weekends of 3pm - 10pm and 8am - 3pm.
* 3pm - 10pm with alternate weekends of 3pm - 10pm and 8am - 3pm.
About you
* Understanding of safeguarding and promoting wellbeing of vulnerable adults.
* Knowledge of safe working practices within the care settings.
* Ability to respond appropriately in challenging situations.
* Experience in Health or Social Care.
* NVQ Level 2 in Health & Social Care (or equivalent).
* Basic IT skills (e.g. Office 365).
While not essential, the following would be advantageous:
* Experience supporting individuals with mental health conditions or physical disabilities.
* Working with multi-agency teams.
* Confidence working independently in a community setting.
* Driving licence with access to a car.
This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate. It is also a regulated activity and will be subject to a Children and Adults Barred List check.
