What the Job Involves As a Leaving Care Worker, you will be a pivotal point of contact, offering reliable support for young people aged 16 to 25 as they leave care. You will coordinate and lead efforts to connect young people with resources from various agencies, ensuring a smooth transition to adulthood. Your role involves direct engagement with young people, helping them achieve outcomes that align with their personal plans and the Care Leavers Strategy. The job requires flexibility, including evening and weekend work, to support individuals at elevated risk levels, manage crises, and maintain accurate records. You will actively participate in meetings, coordinate with partner agencies, and support the Council's transformation initiatives. Skills NVQ Level 3 or equivalent Knowledge of Leaving Care arena (16 to 25) Ability to produce accurate records and reports Strong relationship-building skills Understanding of legislation affecting care leavers Communication and teamwork skills Proficiency in safeguarding and risk management Ability to promote diversity and equal opportunities Driving licence with vehicle availability
