We are excited to offer an opportunity for an experienced and motivated Band 6 Physiotherapist to join our dynamic Community Resource Team, supporting patients within a fast-paced reablement and discharge-to-assess pathway.
This rewarding role focuses on enabling individuals to regain independence, avoid unnecessary hospital admission, and achieve safe, timely discharge from hospital and community services. You will manage a varied and complex caseload, using your clinical expertise and autonomous decision-making skills to deliver high-quality, person-centred care in patients’ own homes and community settings.
As part of a supportive multidisciplinary team, you will work closely with colleagues across health and social care to deliver integrated, seamless care that improves patient outcomes and flow across the system. You will also play a key role in supervising and developing junior staff, students, and support workers, contributing to a positive learning environment.

Main duties of the job
1. Undertake comprehensive physiotherapy assessments and deliver person-centred interventions for patients with a wide range of complex needs within community and home settings.
2. Manage a defined caseload, using autonomous clinical reasoning to assess, treat, and evaluate patient progress, ensuring safe and effective outcomes.
3. Support reablement and discharge-to-assess pathways by promoting independence, preventing hospital admission, and facilitating timely discharge.
4. Work collaboratively within a multidisciplinary team to plan, deliver, and review integrated care, contributing to patient flow and demand and capacity management.
5. Identify and manage clinical risk, escalating concerns appropriately and ensuring safe practice at all times.
6. Provide supervision, guidance, and support to Band 5 staff, support workers, and students, contributing to workforce development and learning.
7. Participate in service development, quality improvement, and audit activities to enhance service delivery and patient outcomes.
8. Maintain accurate, timely clinical records in line with professional, legal, and organisational standards.
9. Contribute to the delivery of a responsive and flexible service, including prioritising workload to meet service demands.

12. Broad range of experience as an NHS rotational Physiotherapist
13. Experience of working as part of a physiotherapy team / MDT team
14. Student or non-registrant staff supervision
15. Involvement in audit, research and understanding quality issues

17. Able to carry out moderate to intense physical effort throughout the working day and carry out concurrent activities
