Overview
An opportunity has arisen within the Integrated Discharge Team (IDT) for an enthusiastic, dynamic and motivated Occupational Therapist to work with the IDT team. This developmental post begins at band 5 and will progress to band 6 in 2 years. You will work alongside a multidisciplinary team of experienced physiotherapists, occupational therapists, nurse specialists, associate practitioners, support workers and administrative staff.
Responsibilities
* Work within both hospital and community settings assessing, managing, and treating patients with the aim of preventing admission to hospital, facilitating timely and safe discharges, and reviewing patients within the community.
* Operate between the office base at Freeman Hospital, RVI and visit patients in their own homes.
* Provide admission‑preventing/proactive discharge planning at the front of house (Assessment Suite and Emergency Department at the RVI) to prevent admissions and/or reduce length of stay.
* Conduct timely and safe discharge planning using the Discharge to Assess (D2A) model, facilitating hospital discharges from Freeman Hospital and RVI, including D2A visits and community follow‑up visits as required.
* Assess and engage patients in the community to attain optimum levels of safety, independence, health and wellbeing.
* Maintain accurate clinical records and be responsible for own clinical practice, including delegation of work to support staff where applicable.
* Work within a 7‑day service from 8:00 AM to 6:00 PM, including weekend and bank holiday commitments.
* Work flexibly and accommodate flexible working arrangements where possible.
Qualifications and Requirements
* Experience at band 5 level working with multidisciplinary teams within hospital or community services.
* Car ownership and valid driver’s licence essential.
* Highly motivated, detail‑oriented with skills in rehabilitation and assessment.
