Overview
We are seeking an experienced Clinical Pharmacist with prescribing qualifications to join our dynamic primary care team. This is an exciting opportunity to work directly with patients and healthcare professionals in a busy GP surgery, providing comprehensive pharmaceutical care and contributing to improved patient outcomes.
The successful candidate will work collaboratively with GPs, nurses, and other healthcare professionals to deliver high-quality clinical pharmacy services, including medication reviews, chronic disease management, and prescribing within their scope of practice.
Main duties
* Conduct comprehensive medication reviews and optimisation for patients with complex medication regimens
* Provide independent prescribing services within scope of practice and competency
* Manage patients with long-term conditions including diabetes, hypertension, COPD, and cardiovascular disease
* Deliver medication adherence support and patient education
* Conduct clinical consultations and follow-up appointments
* Participate in multidisciplinary team meetings and case discussions
* Support practice audit activities and quality improvement initiatives
* Provide medicines information and advice to patients and healthcare professionals
* Maintain accurate clinical records and documentation
* Participate in clinical governance activities and incident reporting
About us
Southbourne Surgery is located nearby to Chichester and Emsworth. There is easy access to the South Downs and to Chichester Harbour. We are a hardworking team who also enjoy socialising.
The current clinical team comprises nine GPs, five nurses, three HCAs, a clinical pharmacist, a pharmacy technician, two paramedics, a Physician Assistant. We also have a number of care coordinators, administrative staff and patient services advisers who provide back office support.
We welcome visits to the surgery to meet the team.

Skills and Knowledge
* Excellent clinical assessment and diagnostic skills
* Strong knowledge of therapeutics and evidence-based prescribing
* Proficiency in clinical examination techniques
* Understanding of primary care systems and referral pathways
* Excellent communication and interpersonal skills
* IT literacy including experience with clinical systems (SystmOne preferred)
* Ability to work independently and as part of a multidisciplinary team
* Knowledge of local formularies and prescribing guidelines
* Research or publication experience
* Leadership and project management skills
Experience
* Minimum 2 years post-registration experience in clinical pharmacy
* Experience in primary care or community pharmacy settings
* Demonstrated experience in medication reviews and optimisation
* Experience working with chronic disease management
* Experience in GP surgery or primary care network
* Experience with clinical audit and quality improvement
* Supervisory or mentoring experience
* Experience in staff training and development
Qualifications
* Master\'s degree in Pharmacy (MPharm) or equivalent
* Registration with the General Pharmaceutical Council (GPhC)
* Independent Prescriber qualification (V300 or equivalent)
* Current DBS check (or willingness to obtain)
* Postgraduate qualification in clinical pharmacy
* Additional clinical qualifications (e.g., injection technique, spirometry)
* Training in motivational interviewing or behavioural change techniques
