About the position
In this role, you will be making sure the delivery of care to our clients is seamless. You will be responsible for coordinating and organising care plans and liaising with healthcare professionals, and care professionals. Ensuring that our clients receive the highest standard of care tailored to their specific needs.
Key Responsibilities:
1. Conduct service reviews and lead quality assurance meetings.
2. Perform daily audits and address concerns promptly.
3. Run incident reports, investigate, and escalate issues as needed.
4. Provide support to Care Professionals with visits and assessments.
5. Maintain accurate records using Home Instead software.
6. Build positive and trusting relationships with clients, families, and care professionals.
Qualifications:
7. Level 3 NVQ in Health and Social Care or equivalent.
8. Proven experience in the care sector.
9. Passion for delivering high-quality care.
10. Excellent communication and rapport-building skills.
11. Confidence in using care management technology.

Additional Information
Full-time position
Salary: £25,000 - £27,000 p.a. (dependent on experience)
