Job overview

We are looking for an enthusiastic and highly motivated Consultant Psychiatrist to join us. He/she will form part of a high calibre consultant peer group in the heart of London and will work in an established multidisciplinary community mental health team in South Westminster.

This is a full‑time 10‑PA replacement substantive consultant psychiatrist post in Victoria PCN to meet the needs of the Service. It has become available owing to the current post holder taking up a different post within the Trust and the second Consultant now going part‑time. There is a higher trainee (ST4‑6) and a junior trainee (CT1‑3) in the team. The post holder will be the clinical supervisor for the higher trainee. There is also a full‑time Specialty doctor in the team who will be supervised by the post holder.


Main duties of the job

* Provide senior medical responsibility for the patients managed by the team and to support all members of the multidisciplinary team (MDT).
* Develop the understanding and capability of the MDT in delivering care informed by the Open Dialogue approach. Open dialogue training is available to the appointee.
* Support recruitment and ensure optimal retention of team members and uphold morale.
* Ensure good service quality and performance.
* Ensure GPs and primary care clinicians are supported to provide mental health care.
* Optimise links with voluntary sector partners to support patient care.
* Ensure training needs of psychiatry trainees and medical students are met as well as supporting training of other junior clinical staff.

Westminster adult mental health service provides a range of mental health services for adults across the age range within the community and in hospital settings. These are integrated health and social care services.

Community services in Westminster are evolving in line with the NHSE Long Term Plan. Most of the service is commissioned by Central London CCG and has undergone a transformation programme redesigned as an integrated care partnership around the four Primary Care Networks (PCN). Two mental health hubs provide an overarching structure in the North and the South of the borough respectively, within which there are functional groupings serving each Primary Care Network (PCN).


Detailed job description and main responsibilities

Clinical responsibilities:

1. Provide senior clinical advice and support to the daily triage, MDT and other regular clinical team meetings.
2. Support the convening of regular Intervision spaces for the team as part of their Open Dialogue practice.
3. Ensure that all referrals are triaged daily and responded to in a timely way.
4. Assessment of new patients and clinical supervision of assessments carried out by other team members, understanding how this can be adapted to be more Open Dialogue informed.
5. Review complex needs patients to ensure appropriate care packages are put in place.
6. Mental Health Act related‑work.
7. Domiciliary visits to provide assessment and treatment services as a member of the CMHT.
8. Domiciliary assessments in response to emergencies.
9. Occasional psychiatric assessments of residents who may be in crisis in another borough.
10. Ensure that physical health needs are addressed.
11. Review and monitoring of the caseload to ensure appropriate care packages are put in place and discharges facilitated to maintain patient flow.
12. Develop good working relationships with local GPs and other providers.
13. Provide mental health advice and support to GPs and primary care clinicians by surgery visits and other means, to assist patient management in the primary care setting.
14. Close liaison with the wards to facilitate admission and discharge planning.
15. Liaison with CRHTT, Psychiatric Liaison Services, AMHP services and Forensic Services in the borough.
16. Develop working relationships with local voluntary sector services in order to support and promote recovery.
17. Following the cross‑cover protocol that has been agreed with another team or colleague which will make sure appropriate consultant cover is in place during absences.

Leadership responsibilities:

1. Leadership role in the quality agenda: identifying areas for improvement and active engagement with QI.
2. Lead the development and understanding of how service processes would need to be adapted to be informed by Open Dialogue.
3. Meet regularly with the team and local service managers to ensure quality, finance and performance targets are met.
4. Support recruitment and ensure optimal retention of team members.
5. Line management and clinical supervision of junior and higher trainees and specialty doctors in the team.
6. Teaching of medical students.
7. Engage in service development, particularly around Open Dialogue transition.
8. Actively link and develop a relationship with the Primary Care Network(s) you are affiliated to, to respond to their developing needs and support the move towards more integrated care.

Person specification


QUALIFICATIONS

* MB BS or equivalent medical qualification.
* Qualification or higher degree in medical education, clinical research or management.
* Additional clinical qualifications.
* Training/Qualification in Open Dialogue.


ELIGIBILITY

* Fully registered with the GMC with a licence to practise at the time of appointment.
* Included on the GMC Specialist Register OR within six months.
* Approved clinician status OR able to achieve within 3 months of appointment.
* Approved under S12.
* In good standing with GMC with respect to warning and conditions on practice.


CLINICAL SKILLS, KNOWLEDGE & EXPERIENCE

* Excellent knowledge in specialty.
* Excellent clinical skills using bio‑psycho‑social perspective and wide medical knowledge.
* Excellent oral and written communication skills in English.
* Able to manage clinical complexity and uncertainty.
* Makes decisions based on evidence and experience including the contribution of others.
* Able to meet duties under MHA and MCA.
* Experience in delivering Open Dialogue in a clinical service.
* Wide range of specialist and sub‑specialist experience relevant to post within NHS or comparable service.


TRANSPORT

* Ability to use public transport (for travel to see patients if indicated).


ACADEMIC SKILLS & LIFELONG LEARNING

* Able to deliver undergraduate or postgraduate teaching and training.
* Participated in continuous professional development.
* Participated in research or service evaluation.
