Join to apply for the Advanced Nurse Practitioner - Hospital at Home role at NHS Scotland. Various hours available. An exciting opportunity has arisen for an Advanced Nurse Practitioner to join the NHS Borders Hospital at Home team. We are looking to recruit several enthusiastic, motivated Advanced Nurse Practitioner (ANP) with excellent communication and clinical practice skills to work within the community setting.
The Hospital at Home (H@H) service provides secondary level care within the patient’s own home or care setting. The ANP within the H@H team plays a key role in the management of acute and complex patients, providing clinical care to patients who would otherwise be in a hospital setting.
Responsibilities
* Be part of the nursing team and undertake staff development and supervision responsibilities as reflected in the key results and as required.
* Work with a senior medical clinician to enhance the ongoing development of the team and staff members.
* Provide clinical care to patients in the community setting, including management of acute and complex cases within the H@H service.
Work pattern
Currently the service is 7 days, 0800-1800. The post holder would work Monday-Friday with weekend working. Part-time salary is pro rata.
