We are looking for
Registered Nurses or AHPs (Allied Health Professionals) to join our team
The Home First Team operates across all adult inpatient areas at the BRI and Weston Hospital.
You will;
1. Use your expert knowledge to coordinate, facilitate and move forward all patient discharges on your wards.
2. Be responsible for a defined caseload working with wards on a rotational basis. You will gain experience in managing patients who have discharges that are “delayed”, “complex” and also work with the admission areas e.g. Emergency Department to prevent admission to the hospital,
3. Supporting and completing initial assessments for Continuing Healthcare Fast Track Assessments (CHCFT) and referral to the Community Transfer of Care Hub (CToCH) for pathway beds.
4. Liaising with the wider MDT, Local Authorities, other Trusts, CToCH and Integrated Care Bureau (ICB) through meetings and the Trust Transfer of Care Hub (ToCH).
5. Ensure local and national polices and guidelines are adhered to, ensuring that the patient and their families are at the heart of everything we do by communicating and supporting them throughout the discharge process.
6. Build key relationships with the ward and therapy teams to ensure good communication and collaborative working relationships to facilitate a timely safe discharge.
