Overview
We are looking for a newly qualified social worker to join the High Wycombe Community Social Work team to complete their AYSE year. Working closely with multidisciplinary teams and supported by dedicated AYSE supervision, you will consolidate learning and transition into a professional role within the Buckinghamshire Council AYSE programme and our s75 agreement with the local authority. AYSE Social Workers carry out a range of social care tasks including assessment of need for people and their carers, review of existing packages of care for service users and carers under the Care Act and section 117 reviews under the Mental Health Act.
Responsibilities
 * Undertake Care Act assessments for adults and their carers who are ordinarily resident in Buckinghamshire and appear to be in need of care and support.
 * Using a strengths‑based approach help adults and their families make best use of community resources to support them to live as independently as possible.
 * Arrange for commissioned services to be put in place where necessary, in line with eligibility and council policy.
 * Review existing packages of care in line with the Care Act and s117 MHA.
 * Work jointly with Social Worker and health colleagues to deliver a supportive and empowering service to adults and their families.
Qualifications
• Newly qualified Social Worker registered with Social Work England.
• Full valid driving licence and access to a vehicle for work.
• Willingness to work within the Buckinghamshire social care framework and to participate in the dedicated AYSE supervision and training programme.
