Band 7 Community Diabetes Nurse
An exciting opportunity has arisen for a Band 7 Community Diabetes Nurse to work with our Integrated Diabetes Nursing service covering Basildon, Billericay, Brentwood, Wickford and Thurrock areas.
Responsibilities
* Provide, support and develop the delivery of high quality, evidence-based Diabetes clinical practice throughout the local community, in health centre clinics and patients’ homes.
* Work closely with the lead Diabetes consultant, GPs, nursing colleagues and other members of the multidisciplinary team.
* Facilitate patient structured education, have a sound knowledge of the principles of diabetes management, and support professional development and advancing with technology. Experience of teaching and good IT skills are essential.
* Travel effectively across the boroughs as part of the role.
Qualifications / Requirements
* Enthusiastic and motivated with excellent communication skills; able to work autonomously, as part of the team and to be an integral member of the wider Diabetes teams.
* Ability to travel across the boroughs; keen interest in professional development and technology.

Start / Induction
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. Induction details include joining on the first Monday of each month, up to two weeks, mandatory training, systems training and equipment allocation. Induction will be held at the Trust head office in Rainham, Essex.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,258 to a maximum of £2,122 per annum pro rata for part time).
