The Care Navigator will also be expected toundertake reception duties as part of the role, supporting the administrativeteam in delivering a polite and professional service to the entitled patientpopulation


Main duties of the job

We are seeking a proactive and organised Care Navigator to join our team, supporting patients and healthcare professionals by efficiently managing appointment bookings, repeat prescriptions, and incoming queries. The role involves signposting patients to appropriate services, maintaining accurate health records, and contributing to public health initiatives. Youll also assist with administrative tasks such as scanning, coding, and data entry, while promoting digital engagement and inclusion. Flexibility, strong communication skills, and a commitment to patient-centred care are essential, along with a willingness to support wider team functions and undertake training as required.


About us

We are a small, friendly, single handed GP practice with just under 4,000 patients and growing. We offer a range of services including general health, maternity and childcare, women's health, and specialist care. We are a paperlight practice working digitally.


Job responsibilities

The following are the core responsibilities of the carenavigator. There may be, on occasion, a requirement to carry out other tasks;this will be dependent on factors such as workload and staffing levels.

* Process and effectively signpost patients to the appropriatehealthcare professional, depending on the presenting condition
* Answer incoming phone calls, transferring calls or dealingwith the callers requests appropriately
* Process patient requests for appointments
* Process repeat prescription requests
* Initiate contact with and respond to requests from patients,team members and external agencies
* Enter SNOMED CT code data using approved templates/coding
* Electronic filing and folder working
* Data entry of new and temporary registrations and relevantpatient information as required
* Input data into the patients healthcare records asnecessary
* Direct requests for information, e.g., SAR,insurance/solicitors letters and DVLA forms, to the administrative team
* Manage all queries as necessary in an efficient manner
* Carry out system searches as requested
* Maintain a clean, tidy, effective working area at all times
* Monitor and maintain the reception area and noticeboards
* Support all clinical staff with general tasks as requested
* Undertake all mandatory training and induction programmes
* Attend a formal appraisal with their manager at least every12 months. Once a performance/training objective has been set, progress will bereviewed on a regular basis so that new objectives can be agreed.

Contribute to public health campaigns (e.g., flu clinics)through advice or direct care

Complete opening and closing procedures in accordance with practice protocols.

In addition to the primary responsibilities, the carenavigator may be requested to:

* Participate in practice audit as directed by the audit lead
* Action incoming emails when necessary
* Scan patient-related documentation and attach scanneddocuments to patients healthcare records
* As required, support in the process of repeat prescriptions,ensuring that they are processed accurately and efficiently
* Order and monitor stationery supplies

Qualifications

* * Educated to GCSE Mathematics & English (C or above) or equivalent
* * Experience of working with the public
* * Excellent communication skills (written and oral)
* * Clear, polite telephone manner
* * Effective time management (planning and organising)
* * Ability to work as a team member and autonomously
* * Good interpersonal skills
* * Ability to follow clinical policy and procedure
* * Polite and confident
* * Flexibility to work outside core office hours
* * Flexible and cooperative
* * Motivated
* * Problem-solver with the ability to process information accurately and effectively, interpreting data as required
* * High levels of integrity and loyalty
* * Sensitive and empathetic in distressing situations

* * Able to communicate effectively and understand the needs of the patient
* * Punctual and committed to supporting the team
* * Experience of working in a primary care environment
* * Experience of working in a healthcare setting
* * Active signposting or Care Navigator training qualification
* * Healthcare qualification (level 2) or working towards gaining equivalent level
* * Competent in the use of Office and Outlook
