Overview
Staff Nurse, Intermediate Care Service (Hospital@Home) role at Temple B'nai Abraham. We are redesigning our community services model and implementing a new Intermediate Care Team. The Nurse will deliver urgent community response services (providing care 8am to 9pm, 7 days a week) and will support independence, dignity and well-being. This is an opportunity to progress in nursing.
The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing care or prevent admission with interventions that can be managed at home.
Responsibilities
* Work with medical and therapy colleagues in the provision of a comprehensive multidisciplinary care service for adults as part of the Intermediate Care (Hospital@Home) Service.
* Support the Team Leader and Deputy in the development and implementation of the service; facilitate integrated working with Local Authorities and third sector organisations to support individuals within their home (where deemed appropriate).
* Take delegated responsibility from the Team Leaders/Deputy for managing patients across a variety of settings, and provide timely clinical assessments and treatment plans.
Qualifications and Skills
* Registered Nurse with current NMC registration (or equivalent).
* Experience in community nursing or integrated care settings preferred.
* Welsh skills desirable: speaking/listening at Level 3 (CEFR B2) or above; not essential for shortlisting.
* Strong communication, assessment, planning and collaboration skills.
