Overview
Provide the clinical pharmacy services to a local hospice via SLA; CF patients in collaboration with the CF pharmacy team and wider CF MDT; and manage, monitor and develop the clinical pharmacy service for homecare initiatives. Deliver high quality pharmaceutical care to meet the needs of palliative care patients within local hospice settings and cross-site LHCH patients under service level agreements.
* Provide a clinical pharmacist role for medical staff and nursing staff on drug use in palliative care within the hospice environment.
* Work with palliative care practitioners and other professionals to facilitate the delivery of national and local health policy.
* Participate in the palliative care education and training programmes of healthcare professionals and students under the guidance of the Senior Clinical Pharmacist - Palliative Care.
* Plan and review the security of medicines at local hospices, monitor drug expenditure monthly with annual reviews, and maintain full control of medicines under the direction of the senior clinical pharmacist for palliative care and Deputy Chief Pharmacist for clinical services.
* Provide a clinical pharmacy service to CF patients in collaboration with the CF pharmacy team and wider CF MDT.
* Under the direction of the Deputy Chief Pharmacist, manage, monitor and develop the clinical pharmacy service for homecare initiatives.
* Undertake Clinical Pharmacist duties including routine monitoring of prescriptions for legality, legibility, safety and appropriateness; implement and develop local prescribing policies and formulary recommendations; formulate and review individual pharmaceutical care plans; assess appropriateness of patients’ own medicines; provide pharmaceutical advice to patients and carers; advise medical and nursing staff on drug choice, dosage, route, duration, interactions and toxicity.
* Monitor and record adverse drug reactions and report to MHRA when necessary; implement patient self-administration schemes; provide discharge information to Community Pharmacists and General Practitioners.
* Advise Ward Managers on safe and secure storage of medicines in line with legislation; record clinical interventions and activities; undertake clinical audit and practice research as required by local/national bodies; investigate medicine-related incidents and minimise future risk.
* Ensure safe dispensing, checking and issuing of medicines from the dispensary during periods of cover in line with Medicines Act, Misuse of Drugs Act and COSHH.
* Support the provision of a specialised clinical pharmacy service to Cystic Fibrosis patients; attend multidisciplinary team meetings, ward rounds and CF outpatient clinics; conduct annual reviews; manage day-to-day CF homecare service; provide clinical pharmacist role for medical and nursing staff on drug use.
* Participate in out-of-hours, bank holidays and weekend provision of pharmaceutical services; be solely responsible for the management of the service at these times, including safety and security of the pharmacy department and controlled storage areas.
* Cover duties of other pharmacists during sickness/absence and undertake other reasonable duties as required.
* Provide a highly specialist clinical pharmacy service to patients of local hospices served by LHCH under SLAs; attend weekly multidisciplinary ward rounds; provide clinical and medicines information; establish communication with Primary Care professionals to facilitate transfer of patient care; provide a clinical pharmacist support role to all hospice patients.
* Attend meetings at the request of the Chief Pharmacist and provide advice to staff and the general public within SOPs for sale of medicines from the pharmacy retail area.
Managerial / Leadership
* Support and develop the annual Pharmacy Business Plan.
* Interpret national legislation, policy or guidance applicable to the specialty and develop and implement pharmaceutical guidelines and/or policies with the Senior Clinical Pharmacist - Palliative Care / Clinical Trials.
* Review NICE Guidance related to palliative care and incorporate recommendations into Trust and Hospice prescribing policies and drug formulary in collaboration with the Drug and Therapeutics Committee.
* Plan, develop and implement prescribing guidelines and protocols for the palliative care team with the Senior Clinical Pharmacist - Palliative Care / Clinical Trials.
* Review and update policies/procedures to ensure compliance and best practice for homecare services in light of national recommendations.
* Liaise with the regional homecare service lead and action relevant communications; review homecare KPIs with the Deputy Chief Pharmacist (patient services).
