What you will do:
* Conduct independent assessments, treatment, and discharge planning for patients within the community rehab team.

* Collaborate with other healthcare professionals to provide comprehensive care for all patients.
Requirements:
* Must have experience post-qualification as a Physiotherapist, ideally within community and rehab settings.
* Must be HCPC registered, ensuring you meet the professional standards necessary for this role.
* Must hold a valid driving licence, as travelling across the community will be an essential part of the role.
