We are looking for an enthusiastic and forward thinking Salaried GP to join our team for 6 sessions per week.

If you enjoy being part of an excellent clinical team, who have regular lunch and learn sessions with peers, monthly dedicated GP meetings with the Partners, protected learning sessions (internal & external learning) and you want to deliver high standards of care to patients living within a demographic footprint that spans from Sedgefield, Wynyard, Fishburn and the Trimdons, then this is the role for you.


Main duties of the job

As a key member of the Practice, you will manage a case load, dealing with a wide range of presenting health care needs in a primary care setting. The practice offers a variety of appointment options, face to face (15 minutes), telephone (10 minutes) and home visits (30 minutes), as well as the opportunity to cover dedicated clinics such as diabetes should you have a specialist interest.

The post holder will work with clinical colleagues to ensure the highest standards of care are provided for all registered and temporary patients. The post holder will also contribute to planning and decision making as a part of the wider practice team.

You are expected to sign repeat prescriptions and deal with queries and re-authorisaton and review of repeat medication as required.

You areexpected to adhere to the practice/ICB formulary where clinically appropriate to doso.

You areexpected to action the administrative work generated as a result of clinicalactivity in a timely and appropriate manner. This includes, but is not limitedto, the filing of blood results and letters.

You areexpected to complete an equitable number of the administrative forms andreports that are generated by the business of running a general practice.

You will berequired to attend courses and where necessary undergo re-assessment processese.g. child protection training updates and CPR / Anaphylaxis training.


About us

Skerne Medical Group is a rural practiceregistered with the Care Quality Commission. The practice provides services toaround 16,700 patients from three locations: Harbinson House in Sedgefield,Carroll House in Trimdon Colliery and Beveridge House in Fishburn.

We area well-established trainingpractice providing high standards of patient focused care. Our practiceteam consists of 7 GP Partners, 4 Salaried GPs and 4 ClinicalPractitioners (who provide our home visiting provision across 5 days), who are supported by an extensive experienced nursing teamalong with a full complement of management and administrative staff who allplay a part in striving to provide the best possible healthcare for ourpatients.

Whilst we work extremely hard, the culture of the practice is friendly and welcoming. Staff wellbeing is at the forefront of our day to day operation and we are currently working towards attaining Better Health at Work Bronze status.


Job responsibilities

You will be expected to:

Make professionally autonomous decisions in relation to presenting problems, whether self-referred or referred internally within the practice

Receive patients with undifferentiated and undiagnosed problems and make assessments of their health care needs

Consult with patients in the surgery, at home visits and via telephone or on line consultations as agreed internally between the practice GPs

Undertake triage calls, triage visits, checking and signing of repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion as required.

Screen patients for disease risk factors and early signs of illness

In consultation with patients and in line with current practice disease management protocols, develops care plans for health

Provide counselling and health education as necessary

Admit or discharge patients to and from caseload and refer them to other care providers internally and externally as appropriate and in line with practice protocols.

Comply with all relevant clinical Governance and Practice policies and protocols.

Assist in the establishment of appropriate systems to manage common chronic medical conditions.

Complete clinically related administrative and non-clinical duties required for the delivery of the service.

Record clear and contemporaneous consultation notes to agreed standards.

Demonstrate commitment to lifelong learning, and audit and effectiveness to ensure evidence based best practice.

Provide active input to the wider Primary healthcare team in the process of health needs research/ base line data collection/clinical audit

Contribute to the evaluation/ audit and clinical standard setting with colleagues.

Record data in patient records systems promptly and accurately and to agreed standards.

Ensure appropriate use of read codes and linking to problem headings.

Contribute to the practice QOF, LIAISE, IIF, DES & PCN achievements

Participate in the collection and collation of statistics towards the preparation of practice reports, performance monitoring and audits.

Participate in the auditing of practice activity as required.

Contribute to the strategic management and development of the practice.
