Martin House has been caring for babies, children and young people with life-limiting conditions from West, North and East Yorkshire for more than 30 years. Every year we care for more than 420 families at our hospice in Boston Spa, in hospitals and in their own homes. We also support more than 150 bereaved families each year.

We are seeking Care Team Nurses and as valued members of the clinical care team, you will join a unique, award winning and vibrant team providing symptom management, palliative and end-of-life care, as well as planned and emergency short breaks both at the hospice and in the community. We offer 1:1 nursing and the opportunity to working as part of the MDT team made up of a variety of skilled members including art and music therapists and a 24 hour medical team.


Main duties of the job

We are looking for compassionate and friendly candidates with a childrens nursing or learning disabilities qualification who have a significant level of hands-on post qualification experience. Our ideal candidates:

* Will need to have knowledge of current legislation relating to children and young people including best practice in safeguarding children.
* Will support the wider team to ensure high standards of holistic care are delivered.
* Be able to assess, plan, implement, evaluate, care and end of life care plans to meet the diverse and individual needs of children, young people and families.
* Has a genuine interest in making a difference, in palliative care with children and young adults.
* Be committed to Martin Houses values Excellence, Respect, Integrity and Compassion
* Be willing to take part in flexible shift work to include early mornings, evenings and weekends.
* Care team nurses will be required work in the community and therefore will need to drive a Martin House pool car, and have access to their own vehicle for business purposes.

Job responsibilities

Overall Purpose of Role:

To provide specialist paediatric and adultpalliative nursing care, in partnership with a multidisciplinary team, bothwithin Martin House Hospice and in the community.

Tasks and duties:

To providehigh quality, clinically effective and evidenced based nursing care andpractice in accordance with Martin Houses policies and standard operatingprocedures.

To provide holistic 1 to 1 care,including activities such as play and recreation as well as attending to thechild or young persons physical and emotional needs.

To act as Nurse in Charge or act asshift lead when required.

Pro-actively support the development ofall Clinical Care Team Members ensuring outstanding standards of care are alwaysdelivered.

Act as a Practice Assessor for studentnurses.

Administermedications and treatments as prescribed, ensuring adherence to safetyprotocols and best practices.

Assess, plan, implement and evaluate end oflife care plans to meet the diverse and individual needs of children, youngpeople and families

Support children and young people to accesscommunity, social and leisure activities.

Tocontribute to assigned governance group including participation in audits,collecting and documenting feedback and research as required.

To pro-actively keep up to date with clinicalcompetencies and e-learning as required.

Develop new clinical skills as appropriate.

Take accountability for own decision making includingthe delegation of tasks to others.

Undertake regular contact calls with assignedfamilies to meet their specific needs.

To participate in general household activities whenrequired, to support the running of the Hospice

To support children and young people in theHydrotherapy pool as required

To plan and deliver Hospice at Home when andwhere required.

Undertake personal and professionalreflective practice to support the revalidation process.

To ensure that care routines andpractice are documented accurately and are clearly communicated.

To ensure accurate and contemporaneousrecord adhering to standards of confidentiality at all times.

To hold each member of the multi-disciplinary team in positiveregard and to foster a culture of respect and consideration.

To work closely with the Care Team Leaderon each shift and inform them of any issues that need resolving.

Toundertake moving and handling techniques, including the use of hoists, pushingwheelchairs and clamping within vehicles.

Supportingchildren and young people in accessing planned community, social and leisureactivities

Non-medical prescribers only: Toundertake prescribing for children and young people with a range of palliativecare needs, within your scope of competence, if authorised to do so.


Person Specification


Qualifications

* Nursing qualification such as childrens nurse, learning disability nurse
* Active NMC Registration with no restrictions on practice
* Experience of working in a care setting, preferably with children and young adults
* Experience of working in a palliative care setting
* Demonstration of nursing children with life limiting conditions, including children with special educational needs.
