Based at the modern Moorgate Primary Care Centre in Bury and the Phoenix Centre in Heywood, our community-focused team delivers tailored diabetes interventions in various settings—ensuring that our patients receive the personalised care they deserve.

Are you an enthusiastic first-level nurse with a passion for diabetes care? We're looking for a dynamic candidate to join our innovative team.
You’ll bring your current NMC registration and up-to-date knowledge of the diabetes specialty, along with experience in managing this complex condition. You’ll have the opportunity to initiate and adjust injectable and oral diabetes medications—empowering patients to manage their health more effectively.
