We are looking for an experienced and motivated Social Worker to join our Patient and Family Support Team, working across our Little Hulton and Heald Green sites to support people living with a life limiting illness, and those close to them.

The successful candidate will be subject to a DBS Check at the level appropriate to the role, the cost of which is covered by the hospice. Any post holder within the organisation will be expected to undertake safeguarding training appropriate to their role and adhere to safeguarding policies and procedures. All staff must work in accordance with their statutory roles and responsibilities in relation to safeguarding in accordance with the Working Together to Safeguarding Children 2023, the Care Act 2014, and the Prevent Duty 2015.
Main duties of the job
To provide specialist palliative care social work interventions to people who access Moya Coles services.
Actively participate in service development and improvement. This will include service evaluation, education and development relevant innovation programs and research.
To act as a source of expert advice and support regarding social work and safeguarding.
To deputise for the Patient and Family Support Team Leader/Advanced Social Worker as and when required.
Job responsibilities
As part of a compassionate, multidisciplinary team, you will provide specialist palliative social work interventions that place the individual at the centre of care. You will bring professional expertise, emotional insight, and a commitment to supporting what matters most to patients and families.
You will:
* Deliver specialist social work assessments and interventions in both inpatient and community settings.
* Provide compassionate, person-centred support to patients, families and carers navigating complex emotional, practical and safeguarding needs.
* Offer expert guidance to colleagues on social work practice, legislation and safeguarding.
* Work collaboratively with nurses, doctors, counsellors, chaplaincy and other professionals to ensure coordinated, safe and effective care.
* Build strong relationships with external agencies to advocate for individuals’ rights, wishes and outcomes.
* Contribute to service development, quality improvement, education and innovation within the hospice.
Qualifications
* A recognised Social Work qualification (Masters / Degree / DipSW)
* Current registration with Social Work England.
Experience
* Significant relevant post-qualifying experience.
* Be committed to and maintain their own continuous professional development in line with Social Work England requirements.
* Demonstrate comprehensive knowledge of relevant legislation and guidance.
* Proactively contribute to the development of services to ensure effective use of resources.
* Demonstrate strong interpersonal skills, both written and verbal.
* Receive and analyse complex information.
* Be proficient in accessing and inputting client information on electronic patient records and other administrative systems.
* Organise and prioritise their workload and time effectively.
* Work flexibly to meet the needs of the service.
* A commitment to ongoing professional development in line with Social Work England requirements.
