Job overview

This post provides an exciting opportunity for an appropriately trained nurse to become an integral part of the established South Yorkshire Regional Services ICC service. The Inherited Cardiac Conditions service (ICC) provides a specialist service for a diverse group of familial conditions that affect the heart and major blood vessels, which can be divided into 5 main sub-specialist areas: Cardiomyopathies, Ion Channelopathies, Aortopathies, Neuromuscular conditions and Familial Hypercholesterolaemia.

The post holder will carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the settings of standards of care. The jobholder will provide specialist advice to a specific patient group and will contribute to the development of the speciality service. The ICC nurse provides specialist nursing care to both newly referred patients and patients under follow-up in the service with the above conditions. This includes not only the management and treatment of patients diagnosed with the condition but also their relatives at risk of developing the condition. This is done through a clinical screening process. ICC patients can develop serious complications as a result of their condition, including MI, heart failure, arrhythmia, vessel dissections and sudden death. The coordination of investigations and interpretation of the results is fundamental to the role.

Main duties of the job

Excellent communication and organisational skills are essential as the successful applicant will act as a source of expertise and knowledge for patients, relatives, and health professionals, and be able to identify patients and relative’s individual specific needs. You will be a point of contact providing support and information for patients with inherited cardiovascular conditions, also providing advice, screening information and supportive counselling as required. You will be responsible for the management of nurse led clinics and your own diverse caseload, working autonomously making independent decisions, completing nurse-led procedures such as ajmaline provocation tests providing feedback to the lead Consultant Cardiologist regarding the decisions made.

If successful your role will include acting as a resource for other health care professionals, providing expert advice and information when required. You will have the ability to work effectively within a team as the role also involves collaboration with the Cardiologists in the care of patients with inherited cardiac conditions and liaising with other specialties including Echocardiography; ECG; Neuromuscular services; Clinical Genetics; Surgeons; Coroners; Geneticists and Paediatric services when required. This also includes facilitating the transition of children aged 13+ years to adult services and working across District General Hospital (DGH) sites to assist in the provision of our established ICC outreach service.

Qualifications

Essential criteria

1. Registered Nurse (Level 1)
2. NMC Registered
3. Non-Medical Prescribing
4. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.
5. Continuous Professional Development
6. Masters degree or working towards (academic level 7)
7. Specialist training relevant to role
8. Advanced Health Assessment qualification (or willing to work towards)

Desirable criteria

9. Relevant post registration qualification relevant to role

Experience

Essential criteria

10. Extensive post registration experience within cardiology/cardiothoracic
11. Relevant experience at Band 6 level
12. Experience of working collaboratively across professional and organisational boundaries
13. Experience of caseload management
14. Ability to demonstrate expert knowledge, training and experience

Desirable criteria

15. Service development and/or change management
16. Audit and standard setting
17. Level 3 Safeguarding
18. Experience of working with ICC patients

Futher Training
